The Special Purpose Examination (SPEX) is a computerized, multiple-choice examination of current knowledge requisite for the general, undifferentiated practice of medicine. The examination is intended for physicians who currently hold, or who have previously held, a valid, unrestricted license to practice medicine in a or Canadian jurisdiction.
 
The SPEX is designed to assess the ability to apply, at a minimally acceptable level, the general medical knowledge considered essential for continued, unsupervised practice by physicians who hold or have held a valid, unrestricted license in a or Canadian jurisdiction.
Appropriate candidates for the SPEX include physicians seeking licensure reinstatement or reactivation after some period of professional inactivity or physicians involved in disciplinary proceedings in which the board determines the need for evaluation. The SPEX is also appropriate for physicians applying for licensure by endorsement who are several years beyond initial licensure.
The SPEX is used by state medical boards to re-examine a physician’s ongoing level of basic medical knowledge. Situations in which a medical licensing board may require a physician to take SPEX include endorsement licensure, reinstatement of a license, or reactivation of a license after a period of inactivity (due to illness, disciplinary action, etc.).
